Hazard Register
Overview
The Hazard Register is a centralised record of all identified hazards which could potentially affect operations, personnel, equipment, or the environment. It serves as a cornerstone tool for systematically tracking, assessing, and mitigating risks.
Various HAZARD TYPES are used to profile risk: top events, hazards / threats, incidents / consequences, and Management of Change events.
Hazards are further CLASSIFIED into relevant categories (e.g. Organisational, Environmental, Human), and assigned to accountable business units and operational locations.
By default, all Top Events are listed on the Hazard Register. At a glance, Safety and QA Officers can see a history of risk levels over time, and filter or search the register for various hazards.
Bowties
A bowtie is a visual diagram that shows how a hazard can lead to a top event (like an accident); it maps both the preventive controls/barriers of threats (on the left side) and the recovery/mitigation measures (on the right side) in one, clear picture.

Hazards / Threats
A threat - or hazard - is any specific event, condition, or circumstance that could directly cause the top event. All hazards - whether added individually or within a bowtie - become part of hazard register.
To open a threat from a bowtie, select the threat to open the context menu, and then select "Open Analysis".
Hazards and threats contribute to overall risk and are the backbone of bowties.
Hazard Register
The Hazard Register is the foundation of effective risk management. A Hazard Register can be imported upon the setup of your SONAR account. You can also add the details of each Hazard Register item individually by selecting the "+Hazard" button in the Hazard Register.
Controls: Barrier and Recovery
Controls are used in both the mitigation of threats and the recovery of incidents.
A BARRIER is a control put in place to prevent a threat and/or top event;
A RECOVERY is a control put in place to handle the recovery of a top event.
Analysis & Evaluation
To evaluate a threat, open it, select "Add New Evaluation" via the + icon in the top menu, and complete the risk assessment questionnaire.

Risk Level
The risk level of a threat / hazard is determined by the evaluation.
A risk can be Low, Medium or High.

Reading the Hazard Register
On the Hazard Register dashboard you will see a list of Top Event Hazards, and a graph gauging risk over time.
